The Transitions in Care Grant Program

 

Note: The 2022 Transitions in Care Special RFP Grant program has closed.  A new Special RFP Grant Program will be announced in 2023.


Coverys is committed to supporting healthcare providers to enhance their culture of safety, reduce risk, and improve patient safety. To evidence this commitment, Coverys, through funding from the Coverys Community Healthcare Foundation (CCHF), established the Special RFP Grant Program to support initiatives that are focused on improving patient safety and clinical outcomes and reducing medical professional liability claims.

2022 grant theme and focus

In alignment with Coverys’ A Dose of Insight — Care Transitions white paper, CCHF chose the specific focus of improving transitions in care for the 2022 Special RFP Grant Program to help concentrate efforts and establish a set of solutions that are likely to have a significant impact in an area that is in need of improvement.

The grant review team focused on proposals with fresh perspectives, data-driven insights, replicable processes, and significant potential to help improve outcomes for patients as they transition from one phase of care to the next.
