The Transitions in Care Grant Program

Coverys is committed to supporting healthcare providers to enhance their culture of safety, reduce risk, and improve patient safety. To evidence this commitment, Coverys, through funding from the Coverys Community Healthcare Foundation (CCHF), has established the Special RFP Grant Program to support initiatives that are focused on improving patient safety and clinical outcomes and reducing medical professional liability claims.

Grant opportunity

CCHF has set aside $400,000 toward its 2022 Transitions in Care Grant Program. Grants of up to $50,000 will be available.

Grant theme and focus

In alignment with Coverys’ A Dose of Insight — Care Transitions white paper, CCHF has selected the specific focus of improving transitions in care for the 2022 Special RFP Grant Program to help concentrate efforts and establish a set of solutions that are likely to have a significant impact in an area that is in need of improvement.

Proposed grants must focus on processes to improve transitions in care

The grant review team is looking for proposals with fresh perspectives, data-driven insights, replicable processes, and significant potential to help improve outcomes for patients as they transition from one phase of care to the next.

Who can apply for a grant?

The Transitions in Care Grant Program is available to all types of applicants for projects specifically addressing transitions in care and that meet our Grant Guidelines.

Dates for submission and selection
  • August 26, 2022–October 14, 2022: Application submission period.
  • Grant awards announced on or before November 30, 2022.

How to submit an application for proposal

To be eligible for consideration for grant funding, applicants must comply with our Grant Guidelines and agree to follow our Reporting Guidelines
