Coverys Community Healthcare Foundation, Inc. (CCHF) is committed to providing resources to US-based healthcare organizations that attempt to advance the practice of medicine and enhance the healthcare experience by creating and testing new processes and procedures or developing strategies focused on improving patient outcomes and mitigating inequities in the access to, and receipt of, quality healthcare. The 2025 CCHF Special Focus Request for Proposal (RFP) Grant Program will focus on:
- Improving diagnostic accuracy and/or improving patient outcomes in physician/office-based healthcare settings, emergency departments, or inpatient healthcare settings.
The work outlined in the grant proposal must focus on advancement in at least one or more focus areas.
Program Information
The 2025 Special Focus RFP Grant Program is now open. This year’s cycle is from August 12, 2025, at 9 a.m. EDT to October 7, 2025, at 11:59 p.m. EDT. Applications received after the grant cycle closes will not be considered. Awards will be announced in early December 2025.To be eligible for consideration for grant funding, applicants must comply with our Grant Guidelines and agree to follow our Reporting Guidelines.

Our 2025 special focus grant ties in with our three-part series of white papers that examine diagnostic error across the following healthcare care settings: emergency department, office-based practice, and hospital inpatient. The first two papers are available now (from links below), and the hospital inpatient paper will be published in 2026.
